Nocturnal hypoxemia, but not hypercapnia, correlates with sleep quality in children.

چکیده
BACKGROUND A correlation has been observed between obstructive sleep events and sleep quality. The aim of the study was to assess if there is also a correlation between nocturnal hypoxemia and hypercapnia and sleep efficiency and sleep fragmentation in children. METHODS Nocturnal pulse oximetry (S(pO(2))) and transcutaneous carbon dioxide (P(tcCO(2))) recordings with simultaneous actigraphy were performed in 38 children with nocturnal hypoxemia and hypercapnia during spontaneous breathing (nocturnal hypoventilation [NH] group), 25 children with partially corrected nocturnal hypoventilation (PC-NH group), and 11 subjects with normal nocturnal gas exchange (no-NH group). RESULTS Sleep efficiency and sleep fragmentation on actigraphy correlated with minimal S(pO(2)) (r(2) = 0.21, P = .004, and r(2) = -0.10, P = .050, respectively) and the percentage of night time with S(pO(2)) < 90% (r(2) = -0.33, P < .001, and r(2) = 0.13, P = .028, respectively) in the NH group. Sleep efficiency and sleep fragmentation also correlated with pulse rate standard deviation (r(2) = -0.42, P < .001, and r(2) = 0.37, P < .001, respectively). No correlation was observed between sleep efficiency and sleep fragmentation and P(tcCO(2)). No correlation was observed between sleep efficiency and sleep fragmentation and S(pO(2)), P(tcCO(2)), and pulse rate in the PC-NH group. Sleep efficiency, sleep fragmentation, and nocturnal S(pO(2)), and P(tcCO(2)) were all normal and not correlated in the no-NH group. CONCLUSIONS In children with nocturnal hypoventilation, nocturnal hypoxemia but not hypercapnia correlates with sleep efficiency and sleep fragmentation on actigraphy.

Chronic obstructive pulmonary disease and sleep.
The control of breathing in patients with chronic obstructive pulmonary disease (COPD) follows the same basic principles as in normal subjects, both awake and asleep, with an expected lower feedback response during sleep. This impacts nocturnal gas exchange and sleep quality most profoundly in patients with more severe COPD, as multiple factors come into play. Hypoventilation causes the most im...
متن کاملHypoxemia and hypercapnia during exercise and sleep in patients with cystic fibrosis.
BACKGROUND In patients with cystic fibrosis (CF), it has been proposed that hypoxemia and hypercapnia occur during episodes of stress, such as exercise and sleep, and that respiratory muscle weakness because of malnutrition may be responsible. METHODS Pulmonary function, respiratory muscle strength, and nutrition were assessed and correlated with the degree of hypoxemia and hypercapnia during...

Introduction: Obstructive sleep apnea syndrome(OSAS) is a common disorder in the general population. Excessive daytime sleepiness (EDS) is a frequent symptom in patients with OSA. Obstructive sleep apnea and its resultant EDS lead to an increased risk of deadly motor vehicle accidents. In this study the relationship between EDS and different polysomnographic variables in patients sufferinf from...
متن کاملNocturnal hypoxemia as a determinant of vigilance impairment in sleep apnea syndrome.
In sleep apnea syndrome (SAS), vigilance impairment is typically associated with highly disrupted sleep, but recently, nocturnal hypoxemia has also been identified as a second pathogenetic factor in patients with a high degree of desaturation. However, although sleep disruption has been demonstrated to play a role in both the propensity to fall asleep and the capacity to stay awake, the role of...

OBJECTIVE To determine the prevalence of nocturnal hypoxemia and its association with pulmonary function, nutritional status, sleep macrostructure, and obstructive respiratory events during sleep in a population of clinically stable children and adolescents with cystic fibrosis (CF). METHODS This was a cross-sectional study involving 67 children and adolescents with CF between 2 and 14 years ...
